Lifeweb Privacy Statement

Data Life Associates, Inc. ("Data Life") has created this privacy statement out of our desire to demonstrate our commitment to your privacy. The following discloses our information gathering and dissemination practices for our applications.

INTRODUCTION

Questions regarding this statement should be directed by email to privacy@datalife.com by phone at (973) 239-7500, or by postal mail at: Data Life Associates, Inc., Attn: Privacy Policy Coordinator, 500 Bloomfield Ave, Verona, NJ 07044.

Any changes in our privacy practices will be posted clearly on our site and will also be sent via email to all affected users.

DATA GATHERED/TRACKED INTO THE SYSTEM

The data entered in the system by authorized users regarding your policies, producers and clients is for your use in operating your business and servicing your clients and producers. The information collected by the application is never sold. Neither will we ever disclose any personal information about you, your clients or producers to any third party for purposes unrelated to servicing you or assuring your satisfaction, without your consent, unless required to do so by law or other regulatory authority.

Information entered into the system by Data Life, including carrier and plan information, is believed to be from reliable sources. However, such information has not necessarily been verified by Data Life or by the related insurance companies.

This system includes several e-mail forms allowing you to contact us regarding customer service, product information, general information, and feedback. The information you provide in these forms, including your name, address, phone number, and account numbers, is used to contact you in order to satisfy your request.

There are several links to other sites within the system. Please note that when clicking on one of these links, you are being transferred to another website. Privacy statements on other sites may differ, and we encourage you to read the privacy statements on other sites you may visit.

ACCESS TO INFORMATION GATHERED/TRACKED BY SYSTEM

Information may be made available to levels below you in the hierarchy if requested by you to do so. Access to view and/or change the policy, client or producer information is restricted based upon the level of access granted and the hierarchical relationship assumed by the user.

Information may also be made available to levels above you in the hierarchy (if existing). The information that is made available will be limited to that information that would, under the normal course of doing business, be made available to that level as the result of the submission of business or contracting a producer. Information not normally required for these activities is not viewable by these levels. Information related to production or policies written through other distribution channels will never be made available without your consent.

SECURITY

Data Life takes seriously the issue of safeguarding privacy online. Great care to secure user's information is exercised.

To prevent unauthorized access, maintain data accuracy, and ensure the correct use of information, we have put in place appropriate physical, electronic, and managerial procedures to safeguard and secure the information we collect online.

Internally, we restrict access to system content to employees and contractors who need access to the information in order to do their jobs. These employees and contractors are very limited in number and are committed to our privacy and security policies.

We provide access to the system via Secure Socket Layer protocol (SSL) to safeguard the transmission of your personal information. This encryption scrambles information as it travels over the Internet so that unauthorized users cannot read it. Your Internet browser already comes equipped with advanced SSL encryption technology.

USE OF IP ADDRESSES & COOKIES

An IP address is a number automatically assigned to your computer whenever you're surfing the Web. Web servers, the big computers that "serve up" Web pages, automatically identify your computer by its IP address. Data Life uses IP addresses to help diagnose problems with our server, to administer our system, and to report aggregate information.

A cookie is nothing more than a small amount of information about you that is stored onto the computer's hard disk. A cookie allows us to recognize only your anonymous computer, not you. Once the cookie is in place, we can recognize future visits by accessing this piece of information. Only temporary cookies are used by the system to maintain the current session state.

OPT-OUT POLICY

If a user discontinues the service, their client, policy and producer data will be removed from the system.
